[1]-First of all Open New Document of Custom size, Height & Width 1200×1200 pixels, Resolution 300 & Mode RGB Color.

[2]-Now Open New Layer & then create the following shape with the help of ‘Rounded Rectangle Tool’ & press right click > make selection.

[3]-Now fill the #’060a60′ color in selection area.

[4]-Now go to Blending Options,& use the following settings

[5]-Now create shape with the help of ‘Rectangle Tool’ & press right click > make selection & fill with #’4677d0′ color.

[6]-Now create another shape with the help of ‘Rounded Rectangle Tool’ & press right click > make selection & fill with White color.

[7]-Now go to Blending Options,& use the following settings

[8]-Now create the following lines with the help of Pen Tool.

[9]-Now create shape with the help of ‘Pen Tool’ & press right click > make selection & fill with White color.

[10]-Now set the Opecity like as shown below.

[11]-Now create the following stroke with the help of Stroke Path.

[12]-Now select the ‘Horyzontal Type Tool’ & write the Text.

[13]-Now your ‘Final’ image should look like as shown below.
